Simply complete and return an application or visit any location to apply for your Bank Star Plus check card.  When approved, you will receive your Bank Star Plus check card by mail.  Your PIN (Personal Identification Number) will arrive in the mail separately a few days later.  For your account security, you will need to contact the bank to activate your card.  You may use the PIN that has been assigned to your account or come by the bank and select a PIN to start using your Bank Star Plus check card immediately.
